Appointments

Office hours are Monday through Thursday 8:00am to 4:30pm and Friday 8:00am to 3:00pm.

Appointments can be scheduled by calling the office during normal business hours at (352) 331-3583

New patients are asked to have their paperwork filled out prior to your appointment. If this is not done then we request that patients arrive at least 20 minutes early to fill out this paperwork. You may download and print out the forms and complete paperwork from our forms page. Please bring the forms and insurance card(s) to your appointment.

New and referred patients need to bring ALL records and films from ALL other treating physicians pertaining to the problem for which they are being seen. All x-rays, MRIs and CT scans can be on films or CDs. Please advise the office if these records and films cannot be obtained. It is the patient's and referring doctor's responsibility to send and/or fax all the above to our office prior to your appointment.

Cancellations

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We do reserve the right to charge patient’s for missed appointments.
